Screening and Contact Tracing Process
Dear GMHA Membership,
As per the City of Hamilton, COVID-19 screening and contact tracing needs to occur for each on-ice session. A member of each family entering the arena is required to complete screening and contact tracing on behalf of the family.

The screening and contact tracing questions can be found by scanning the QR code on Glanbrook posters located at the entrance of each arena or by the link found as a  banner on the Top of the Glanbrook Webpage.

The information to be provided is as follows:

1) Current Location of event
2) First and Last Name
3) Contact Phone Number
4) Vaccination status or tested positive in previous 90 days and have been cleared
5) Screening questions (symptoms, travel, required isolation, positive test in last 10 days)

Temperatures will NOT be taken.

Completed forms are held in a secure online database accessible only to the
Glanbrook Rangers representative and will only be shared with the City of
Hamilton Public Health Department in the event of an outbreak and contact tracing
is required.
