Permission to Skate & Non Resident Passport Forms!!!!

As Hockey Canada struggles to fix the registration website, we have to proceed with handing out the Permission to Skate & Non-Resident Passport forms for AAA & AA tryouts.  These forms can be picked up on the following dates and times. 

April 6th 6:30pm to 8:30pm *at Fortino's (Upper Centennial) community room* 
April 8th 6pm to 8pm *Glanbrook Arena*
April 9th 6pm to 8pm *Glanbrook Arena*


Permission to Skate forms
please email the following information (24 hours prior to pick-up date) to [email protected]       

Player’s name:
Date of Birth:
Previous season team including association:
Specify which team the player intends to tryout for under this permission.  

Non Resident Passport Forms
Note: under a NRP a player is allowed to the next closest “AA” centre or the next closest “A” centre. You have to choose one or the other. If you are uncertain to which is your next closest centre, please inquire. If applicable, please email the following information (24 hours prior to pick-up date) to [email protected]
