Permission to Skate Forms, Non Resident Passport Forms and Permission to Tryout for Junior Forms
The above mentioned forms can be picked up on the
following dates and times in the Glanbrook Minor Hockey Room:
April 2nd 6:30pm to 8:30pm

April 4th 10am to 12noon (Cancelled, to be rescheduled)
April 7th 6:30pm to 8:30pm
Please complete the following steps to receive your forms:
Step 1 (For the Junior form, go to Step 4)
Register Online
In order to receive a PTS and/or a NRP form(s), you will be required to register online and pay in full prior to receiving these forms. Registration & payment is not required for the Junior Forms.
Step 2 (Permission to Skate Form)

Step 3 (Non – Resident Passport)
Note: under a NRP a player is allowed to the next closest “AA” centre or the next closest “A” centre. You have to choose one or the other.
