Permission to Skate Forms, Non Resident Passport Forms and Permission to Tryout for Junior Forms

The above mentioned forms can be picked up on the following dates and times in the Glanbrook Minor Hockey Room:  

April 2nd 6:30pm to 8:30pm   (Cancelled, to be rescheduled)
April 4th 10am to 12noon  (Cancelled, to be rescheduled)
April 7th 6:30pm to 8:30pm




Please complete the following steps to receive your forms:  

Step 1 (For the Junior form, go to Step 4)
Register Online

In order to receive a PTS and/or a NRP form(s), you will be required to register online and pay in full prior to receiving these forms.  Registration & payment is not required for the Junior Forms.   

Step 2 (Permission to Skate Form)
If applicable, please email the following information (24 hours prior to pick-up date) to [email protected]    
Player’s name:
Date of Birth:
Previous season team including association:
Specify which team the player intends to tryout for under this permission.  

Step 3 (Non – Resident Passport)
Note: under a NRP a player is allowed to the next closest “AA” centre or the next closest “A” centre. You have to choose one or the other. If you are uncertain to which is your next closest centre, please inquire. If applicable, please email the following information (24 hours prior to pick-up date) to [email protected]
