PERMISSION TO SKATE FORMS!!!!!

Permission to Skate forms can be picked up on the following dates and times in the Glanbrook Minor Hockey room: 

Friday, March 28 between 6:30pm - 7:30pm
Saturday, March 29 between 10:00am - 12:00pm 
Monday, March 31 between 6:30pm - 8:00pm
Saturday, April 12 between 1:30pm - 3:30pm.  (Final Day)

To help speed up the process, can you please email the following details to  [email protected] and/or Lori Smith at [email protected]

Previous Team and Association

When picking up the PTS forms, can you please have proof of address with you.
